This form is only for books on hand at the Appleton Library. Use the ILL Request Form to request an interlibrary loan.

You can search the library's online catalog to find the book(s) you want before completing this form. The catalog site will ask for the library name: Appleton Library. No password is needed.

You may have a maximum of 10 books checked out at one time.

After you submit this form, a staff member will contact you to schedule a time to pick up your book outside the library.

Pick up hours are:

Tuesdays, 10 am - 1 pm

Thursdays, 6:00 pm - 8:00 pm
